Does a Penile Implant Make You Bigger? ED Implants vs Cosmetic Implants

10 min read

No, a penile implant for erectile dysfunction does not make the penis bigger than it was before. It restores firmness, and the erection usually matches your stretched length before surgery. A cosmetic silicone implant is a different device, because it adds girth but does not treat erection problems.

Medically reviewed by Assoc. Prof. Dr. Tuncay Taş, urologist. Updated November 2026.

Adverts and search results often mix these two implants. This guide separates them and explains why some men feel shorter after surgery. It also shows what can protect your size.

Two very different implants

The phrase "penile implant" can describe two separate devices. They sit in different places, serve different goals and suit different men.

Feature

ED implant

Cosmetic implant

Main purpose

Firm erections on demand

More girth

Where it sits

Inside the erectile bodies

Under the skin of the shaft

Treats ED?

Yes

No

Adds length?

No

No

Adds girth?

Not beyond your natural size, in most men

Yes, and it stays

Usual candidate

A man whose ED no longer responds to other treatment

A man with healthy erections and a girth goal

So your first question to any clinic should be simple: which implant do you mean? The answer changes the goal, the risks and the realistic outcome.

What an ED implant does to size

An ED implant, also called a penile prosthesis, replaces the failed erection mechanism. The NIDDK describes it as an option when other treatments have not helped. It fills the erectile bodies from the inside, so it works with the space your body already has.

During surgery, Assoc. Prof. Taş measures the inside of each erectile body and selects cylinders to fit. He aims for the fullest size that remains safe.

Yet the device cannot stretch those chambers beyond their limit. So the erection usually comes close to your stretched length before surgery.

Inflatable and malleable devices

The type of device affects how the penis looks, although neither type works as an enlargement. Our penile implant surgery page compares all the models in detail.

  • Three-piece inflatable: cylinders fill from a reservoir, so the penis feels firm when inflated and soft when deflated.

  • Girth-expanding cylinders: the Titan penile implant is one example, and these cylinders widen as they fill.

  • Length-and-girth cylinders: some models can also expand slightly along their length, though this does not create new erectile tissue.

  • Malleable rods: the malleable penile implant keeps a fixed size and does not widen on demand.

We do not quote a typical length change in centimetres here, because public health sources do not give one. Moreover, your own result depends on your tissue, your earlier history and the device.

Why the erection can feel shorter

Some men feel their erection has lost length after an implant. In most cases, several separate effects explain that feeling. The device itself is only one of them.

Reason

What happens

What can help

Long-standing ED

Tissue loses stretch over the years without firm erections

An earlier visit to a urologist

Memory of past erections

Men compare with erections from years ago

Measuring stretched length before surgery

A softer head

The cylinders stop before the glans, so it can stay softer

Discussing tablets with your urologist

Scar tissue

Scarring limits how far the chambers stretch

Careful sizing and an experienced surgeon

Prostate surgery or Peyronie's

Both can shorten the penis before any implant

Measuring before surgery

The role of a softer glans

The implant sits in the shaft and does not reach the head of the penis. Therefore, the glans fills through your natural blood supply.

It may feel less full than you remember. This can make the penis look a little shorter, even at the expected shaft length.

Some urologists suggest an ED tablet to improve glans fullness in selected men. Ask your own doctor whether this suits you, since heart conditions and some medicines rule it out.

Does an implant keep shrinking the penis?

An implant does not normally cause ongoing shrinkage. In fact, the cylinders hold the chambers open from the inside. So a new, steady loss of length after healing deserves a review, because scar tissue or a device problem may lie behind it.

Replacement surgery raises a separate question. Scar tissue around an old device can make the next fit harder.

For that reason, ask your surgeon how a future replacement could affect your length. Our penile implant revision page explains that operation.

Why waiting can matter

Without firm erections, the penis gets little regular stretch. Over time, this can reduce elasticity and make a later operation harder.

So if your treatments have stopped working, do not wait for years without a review. You can do this even if you remain unsure about surgery.

This does not mean you must rush into an implant. Instead, see a urologist while you still have choices, and read our erectile dysfunction treatment page for the other steps.

What a cosmetic silicone implant does

A soft silicone girth implant sits just under the skin of the shaft, outside the erectile bodies. It adds permanent width to the soft and erect penis, so it works as a size procedure rather than a treatment for ED.

Because it sits under the skin, this implant cannot make erections firmer. It does not lengthen the erectile bodies either.

For a closer look at these devices, see our post on silicone sleeve implants. Our penile girth enhancement page compares the implant with filler and fat transfer.

Can both implants work together?

Yes, in selected men. A surgeon can place a soft silicone implant for girth together with an inflatable implant for firmness. However, a combined plan means more surgery and more tissue to heal, so it needs a clear reason.

Infection risk also deserves attention, because an infected device may need removal. Assoc. Prof. Taş therefore checks your skin, blood sugar and earlier surgery first.

What about extra length?

No implant adds length to the erectile bodies. If length is your main concern, the options lie elsewhere, and each has limits of its own.

  • Ligament release: this surgery reveals hidden shaft, so the main change appears at rest. See our ligamentolysis guide for what it can and cannot do.

  • Fat pad contouring: a thick pubic fat pad can hide shaft, and reducing it brings that shaft back into view.

  • Traction: a device offers a small change at most, and it takes months of daily wear.

Also, a longer cylinder is not a safe way to chase size. A cylinder that does not fit can press through the tissue, cause pain or fail. So correct sizing protects you.

What you may notice in the first year

Size impressions often change as healing goes on. Swelling can make the penis look larger at first. Then it fades, and some men worry that they have lost length.

  • First weeks: swelling and bruising hide the true result.

  • After activation: the first uses can feel tight, and the erection may look narrower.

  • First months: regular use often makes the erection feel fuller.

  • Around one year: most men have a stable picture of their result.

So judge your result after healing, not in the first weeks. Your own measurements from before surgery make this comparison fairer.

Does the soft penis look different?

A deflated inflatable implant usually looks natural, and it stays private. However, it does not hang exactly as before, because the cylinders still sit inside.

A malleable implant stays semi-firm all the time, so it can look fuller at rest. Even then, it has not grown.

Does body weight change the look?

Yes, it can. A thick pubic fat pad hides the base of the shaft, with or without an implant.

Weight gain can therefore make an implant look shorter. Weight loss sometimes reveals more shaft, and it also lowers some surgical risks.

Can you protect your size around surgery?

You cannot change your anatomy, but several steps help you keep what you have. Your urologist will tell you which ones apply to you.

  1. Measure before surgery: stretched length gives you an honest reference point, and it helps with expectations later.

  2. Treat ED in good time: an early review can limit the loss of elasticity.

  3. Control your health: better blood sugar and no smoking support healing and lower infection risk.

  4. Follow the device routine: regular inflation, once your surgeon allows it, helps the space around the cylinders settle.

  5. Give it time: some men notice a fuller erection over the first months as they use the device.

Our penile implant recovery timeline explains when each stage usually happens.

Which implant fits your goal?

The right choice starts with your main problem, not with the device name. Assoc. Prof. Taş begins by asking what bothers you most, then he checks how your erections work.

Your main concern

Option to discuss

Be realistic about

Erections fail despite treatment

ED implant

Firmness returns, size does not grow

Healthy erections, thin shaft

Girth method

Width changes, length does not

Healthy erections, short look at rest

Ligament release or contouring

Erect gain stays modest

ED and a girth concern

ED implant with a soft implant

More surgery and healing

Normal size, ongoing worry

A reassuring talk first

Surgery may not ease the worry

Size worries deserve a careful answer too. The NHS describes body dysmorphic disorder as a treatable condition, so ask your doctor for help if these thoughts fill your day.

What about injections for size?

Permanent injectables are not an alternative to an implant. The US FDA states that injectable silicone is not approved for any aesthetic procedure, and it can cause serious harm. Hyaluronic acid filler is a different material, and you can read about it in our penile filler guide.

Risks to weigh

Both implants involve real surgery, and neither should be a casual decision. Most men recover well, yet you deserve to hear the possible problems plainly.

  • Infection: an infected implant often has to come out.

  • Mechanical failure: an inflatable device can wear out and need replacing, as our post on how long a penile implant lasts explains.

  • Loss of natural erections: an ED implant changes the erectile tissue, so natural erections rarely return if you later remove it.

  • Change in sensation: numbness or altered feeling can occur, especially early on.

  • Cosmetic implant problems: shifting, a visible edge, tight skin or removal can occur.

  • Disappointment: the result may differ from what you pictured, especially if you expected more size.

That loss of natural erections matters for this topic. An ED implant is not a sensible route to size if your erections are healthy. It would permanently replace something that still works.

What does each implant cost?

Prices depend on the device, and a written quote follows the consultation. These are the guide prices from Assoc. Prof. Taş's published list for Istanbul.

Device

Main purpose

Guide price

Malleable prosthesis

Firmness

$5,000 to $7,000

Inflatable prosthesis

Firmness

$6,000 to $12,000

Soft silicone implant

Girth

$10,000 to $11,000

By comparison, the same list shows inflatable devices at $20,000 to $35,000 in the United States. In the United Kingdom, it shows £14,000 to £22,000. Medical cover for an ED implant varies by plan and country, while insurers rarely pay for cosmetic girth surgery.

Questions to ask before you book

These questions keep the conversation on size honest.

  • Which type of implant do you recommend for me, and what is its purpose?

  • Will you measure my stretched length and record it before surgery?

  • What erect size should I realistically expect with this device?

  • Do my earlier treatments or surgeries limit the result?

  • What happens if the device fails or becomes infected?

  • Who looks after me once I am back home?

A clear answer to each question is a good sign. On the other hand, be wary of any clinic that promises a bigger penis from an ED implant.

Travelling from the US, Canada or the UK

Many men compare costs abroad, and Istanbul is one option to weigh. Even so, the NHS advises checking the surgeon's training and the aftercare plan before any treatment abroad.

  • Time in Istanbul: plan about a week, so that a wound check happens before you fly.

  • Your records: keep the implant card with the brand, model and cylinder length.

  • Back home: tell your own doctor about the device and agree who will review you.

For a practical plan, see our page on penile implant surgery for patients from the US and Canada.

When to see a urologist

See a urologist if tablets and other treatments no longer give you a usable erection, or if size worries affect your relationships. Book sooner in these situations.

  • Your penis has become shorter or more curved over recent months.

  • You have an implant and notice redness, fever or growing pain.

  • Your device has stopped inflating or deflating properly.

  • You had an injection elsewhere and now feel lumps or hardness.

To discuss your goal with Assoc. Prof. Taş, book a consultation or message the team on WhatsApp. He will explain what each implant could realistically change for you.

Frequently asked questions

Does a penile implant make you bigger?

An ED implant does not make the penis bigger than it was before surgery. It restores firmness, and the erection usually matches your stretched length. A cosmetic silicone implant adds girth, but it does not treat erection problems.

Does a penile implant add length?

No. An implant fills the erectile bodies you already have, and it cannot lengthen them. Some cylinders can expand slightly along their length, yet they stay within the space of those chambers.

Why does my penis seem shorter with an implant?

Several things can explain it. Long-standing ED reduces elasticity, the glans may stay softer and men often compare with erections from years ago. Scar tissue and earlier prostate surgery can also play a part.

What is the difference between an ED implant and a penis enlargement implant?

An ED implant sits inside the erectile bodies and creates firm erections. A cosmetic enlargement implant sits under the skin of the shaft and adds girth. Only the first treats erectile dysfunction.

Can I have a penile implant just for size?

An ED implant is not suitable for size alone, because it changes the erectile tissue and natural erections rarely return. If your erections are healthy, your urologist can discuss a girth method instead.

Can a girth implant be added to an inflatable implant?

Yes, in selected men. A surgeon can place a soft silicone implant with an inflatable device, either together or later. This means more surgery, so the plan needs careful assessment of infection risk.

Will my erection get bigger over time with an implant?

Some men notice a fuller erection during the first months as they use the device regularly. However, the erection still stays within the natural limits of the erectile bodies, so expect a small change at most.
