Penuma and Himplant: How Silicone Sleeve Implants Compare with Fillers and Fat
Penuma and Himplant are brand names for a soft silicone sleeve that a surgeon places under the skin of the penis. Unlike filler, the sleeve does not fade, so it adds permanent girth and a fuller shape at rest. However, it needs surgery, carries surgical risks and can only come out in a second operation.
Medically reviewed by Assoc. Prof. Dr. Tuncay Taş, urologist. Updated October 2026.
This guide explains how sleeve implants work and what they change. It also compares them with filler and fat transfer, so you can weigh permanence against reversibility. Finally, it lists the questions worth asking any surgeon before you agree to an implant.
What is a silicone sleeve implant?
A silicone sleeve implant is a curved piece of soft, medical-grade silicone. It wraps over the top and sides of the shaft, just under the skin. As a result, the penis looks and feels thicker, both at rest and during erection.
The implant sits outside the erectile chambers. So it does not change how erections work. That also sets it apart from a penile prosthesis, which goes inside the erectile chambers to treat erectile dysfunction.
Penuma, Himplant and generic sleeves
Penuma became the first widely known sleeve implant in the United States. Later, its maker began using the name Himplant. So, in most conversations, the two names describe the same product line.
Other companies also make soft silicone girth implants. Their shapes, sizes and fixation methods differ slightly. Still, they share the same idea: a soft silicone layer under the skin of the shaft.
A note on FDA clearance
In the US, Penuma reached the market through the FDA 510(k) pathway. That route compares a device with similar products already on the market, which differs from full approval based on large clinical trials. So ask any clinic what data supports the device it uses.
Sleeve implant vs filler vs fat transfer
All three methods add girth, yet they behave very differently over time. The table compares them, using the guide prices Assoc. Prof. Taş publishes for Istanbul.
Feature | Hyaluronic acid filler | Fat transfer | Silicone sleeve implant |
Material | Hyaluronic acid gel | Your own fat | Soft medical-grade silicone |
Anaesthesia | Local | Sedation or general | General or spinal |
How long it lasts | About 12 to 18 months | Years, after early partial loss | Permanent |
Reversible? | Yes, an enzyme dissolves it | No | Only by surgery |
Back to desk work | Next day | 2 to 3 days | About 1 week |
Sex again | After 1 to 2 weeks | After about 6 weeks | After about 6 weeks |
Guide price in Istanbul | $1,500 to $3,000 | $3,000 to $5,000 | $10,000 to $11,000 |
In short, filler lets you test a change, fat uses your own tissue, and the implant gives a stable, lasting result. For a deeper look at each girth option, read the penile girth enhancement guide.
What a sleeve implant changes, and what it does not
Most men choose a sleeve for girth, but the change shows most clearly at rest. So it helps to set expectations for each state.
What you notice | Effect of a sleeve implant |
Flaccid girth | Clearly thicker and fuller |
Flaccid length | Often looks longer, because the penis hangs fuller |
Erect girth | Thicker, set by the implant size |
Erect length | No meaningful change |
Erections | No change, because the implant stays outside the erectile tissue |
Feel to touch | Firmer under the skin than natural tissue |
If length is your main goal, a sleeve alone will not meet it. Instead, read the penis lengthening surgery guide, which explains ligament release.
How the operation works
The steps below describe a typical sleeve implant with Assoc. Prof. Taş in Istanbul. Each step follows a consultation and a written plan.
Measurement: first, he measures your girth and length and agrees on an implant size with you.
Anaesthesia: next, you sleep under general anaesthesia, or the lower body goes numb with a spinal.
Placement: then, through a small incision near the base, he creates a space under the skin and positions the implant.
Closure: finally, he closes the wound in layers and applies a light dressing.
The operation usually takes one to two hours. Afterwards, most men stay in Istanbul for five to seven days, so the first wound check happens before the flight home.
Combining a sleeve with a penile prosthesis
Some men need both more girth and firmer erections. In that case, a sleeve can sit alongside an inflatable penile prosthesis. Our post on enlargement in men with a penile prosthesis explains how surgeons plan that pairing.
Recovery after a silicone sleeve implant
Recovery follows a predictable pattern, although every man heals at his own pace. The milestones below give a typical timeline.
Days 1 to 3: swelling and soreness, eased by simple pain relief.
Week 1: most men return to desk work and light walking.
Weeks 2 to 4: swelling falls, yet heavy exercise stays off limits.
About 6 weeks: sex and sport usually resume once the wound heals.
Months 2 to 3: the final shape settles as the tissue softens around the implant.
After you fly home, the team follows progress by video call. Also, you receive written aftercare advice in English and a direct line for questions.
Risks of sleeve implants
A sleeve implant is a real operation, so it carries real risks. Published reports show that most men do well. Even so, some need further surgery, and a few need the implant removed.
Swelling and bruising: common in the first two weeks.
Fluid collection (seroma): may need drainage.
Infection: uncommon, but it often means removing the implant.
Shape problems: the implant edge may show, or the implant may shift.
Scar tightening: firm tissue can form around the implant over time.
Changes in sensation: usually temporary.
For these reasons, Assoc. Prof. Taş does not offer a sleeve to men with poorly controlled diabetes, active infection or very thin shaft skin. He also refers men whose worry about size far outweighs their anatomy for support first.
What happens if a sleeve implant has to come out?
Removal is the main trade-off of a permanent implant. So it deserves a clear plan before surgery, not after a problem appears.
A surgeon usually removes a sleeve through the original incision. If infection caused the problem, he first treats the infection and may wait before any further step. As a result, removal can mean a second stay and extra time off work.
After removal, the skin of the shaft may look looser or slightly uneven for a while. In most men it settles over several months.
However, firm scar tissue sometimes remains, and a later girth treatment then becomes harder. For that reason, Assoc. Prof. Taş explains the removal pathway, and its cost, at the first consultation.
How long does a sleeve implant last?
The silicone itself does not dissolve or wear down like filler. So, in a man who heals well, the added girth stays for years. Unlike an inflatable prosthesis, the sleeve also has no pump or fluid, so no mechanical part can fail.
Even so, the body can change around the implant. For example, weight gain, scar tightening or thinning skin can alter how the shaft looks later. Therefore, a yearly check, in person or by video, helps catch small changes early.
Living with a sleeve implant
Once the tissue heals, daily life usually feels normal. Still, men often ask the same practical questions.
Sport and cycling: most activities return after about six weeks; long rides on a hard saddle may need a softer seat at first.
Condoms: a thicker shaft may need a larger condom size.
Airport security: soft silicone contains no metal, so it does not trigger metal detectors.
Scans: silicone does not stop you from having an MRI, but tell the radiology team about the implant.
Check-ups: report any new swelling, redness or change in shape promptly.
In addition, keep your device card with your medical records. Then any future doctor can see exactly which implant you carry.
Who is a good candidate?
A sleeve implant suits a narrow group of men. In particular, the best candidates share these traits.
A clear girth goal: you want more thickness, not mainly length.
Good general health: no active genital infection and stable blood sugar.
Healthy shaft skin: enough skin to cover the implant without tension.
Realistic hopes: you accept a natural, proportionate gain.
Certainty: ideally, you have already tested girth with filler.
Many men who ask about implants already fall within the normal size range. In that case, Assoc. Prof. Taş says so plainly, because reassurance sometimes helps more than surgery.
What does a silicone girth implant cost?
With Assoc. Prof. Taş, a soft silicone girth implant costs $10,000 to $11,000 in Istanbul. By comparison, the guide range runs about $22,000 to $30,000 in the United States.
Where | Silicone girth implant, guide range |
Istanbul, Assoc. Prof. Taş | $10,000 to $11,000 |
United States | $22,000 to $30,000 |
United Kingdom, private | £16,000 to £20,000 |

Questions to ask before any sleeve implant
A careful surgeon welcomes these questions. In fact, the answers show whether a clinic offers medical care or simply sells a product.
Which implant goes in? Ask for the brand, size and a device card with the lot number.
Who operates? Ask whether a urologist assesses you and performs the surgery.
What if I need removal? Ask who removes the implant and what that costs.
What gain can I expect? Ask for a target in centimetres, not a promise.
How does follow-up work? Ask how the team handles a problem once you are home.
The NHS also advises checking a surgeon's registration and aftercare plan before any procedure abroad. To compare real outcomes, browse the penis enlargement before and after gallery.
When to see a urologist
See a urologist before choosing any implant. Also, seek help promptly in the following situations.
You already have a sleeve implant and notice redness, fever or growing pain.
The implant edge has started to show, or the shape has changed.
You had an injection elsewhere and now feel lumps under the skin.
Your erections have become weaker.
To discuss whether a sleeve suits you, book a consultation with Assoc. Prof. Taş or message the team on WhatsApp. If a reversible first step makes more sense, he will explain the penile filler option instead.
Frequently asked questions
Are Penuma and Himplant the same thing?
They come from the same maker. Himplant is the newer brand name for the Penuma silicone sleeve implant. Other companies also make soft silicone girth implants that work in a similar way.
Is a silicone sleeve implant permanent?
Yes. The silicone does not break down, so the added girth stays. Even so, a surgeon can remove the implant in a second operation if a problem appears.
Does a sleeve implant add length?
It mainly adds girth. The penis may look longer at rest because it hangs fuller, but the implant does not change erect length in a meaningful way.
Will a sleeve implant affect my erections?
It should not, because the implant sits under the skin and outside the erectile tissue. If you already have erectile dysfunction, it can sit alongside an inflatable penile implant.
How long is recovery after a silicone sleeve implant?
Most men return to desk work in about a week. Sex and sport usually wait about six weeks, until the tissue heals around the implant.
How much does a silicone girth implant cost in Istanbul?
With Assoc. Prof. Tuncay Taş, a soft silicone girth implant costs $10,000 to $11,000 in Istanbul. In the United States, the guide range is about $22,000 to $30,000.
Should I try filler before an implant?
Many men do. Filler lasts about 12 to 18 months and an enzyme can dissolve it, so it lets you test how more girth feels before a permanent step.
