Is Penis Enlargement Permanent? How Long Each Method Lasts
Some penis enlargement results are permanent, and some are not. A soft silicone girth implant and a ligament release give lasting change, while filler fades over 12 to 18 months and part of transferred fat disappears early. Pills, creams and stretching exercises have shown no lasting gain at all.
Medically reviewed by Assoc. Prof. Dr. Tuncay Taş, urologist. Updated October 2026.
So the honest answer depends on the method, your body and your aftercare. This guide walks through each option in turn. Also, it shows what "permanent" really means for each one, so you can weigh results against risk.
What does "permanent" mean in penis enlargement?
Clinics use the word "permanent" in different ways. For that reason, it helps to split it into three separate questions.
Does the material stay? Filler breaks down, while a silicone implant does not.
Does the gain stay? Scar tissue, weight gain or fat loss can still shrink a result.
Can you undo it? Some lasting methods are hard to reverse, so permanence cuts both ways.
In short, "permanent" can be good news or bad news. A result you love should last, but a result you dislike should be easy to reverse. That trade-off sits at the centre of every choice below.
How long each method lasts: a side-by-side view
The table below compares the methods Assoc. Prof. Taş uses in Istanbul. Prices come from his published price list, and they show guide ranges per treatment.
Method | Main goal | How long it lasts | Can it be reversed? | Guide price in Istanbul |
Hyaluronic acid filler | Girth | About 12 to 18 months | Yes, an enzyme dissolves it | $1,500 to $3,000 per session |
Fat transfer | Girth | Years, after early partial loss | No | $3,000 to $5,000 |
Soft silicone girth implant | Girth | Permanent | Only by surgery | $10,000 to $11,000 |
Suspensory ligament release | Visible length at rest | Permanent, with traction aftercare | No | $3,500 to $6,000 |
Traction or vacuum device | Small length support | Fades if you stop | Not needed | Device cost only |
Pills, creams, exercises | None proven | No lasting gain | Not needed | Not recommended |
For comparison, the same treatments usually cost two to three times more in the United States and the United Kingdom. For instance, a silicone girth implant runs about $22,000 to $30,000 in the US.
Temporary by design: hyaluronic acid filler
Hyaluronic acid occurs naturally in human skin. So when a urologist places this gel under the skin of the shaft, the body breaks it down slowly. Most men see the volume fade towards their starting point over 12 to 18 months.
That short life is not a flaw; instead, it works as a safety feature. If a lump forms or you dislike the shape, an enzyme can dissolve the gel within days. For this reason, Assoc. Prof. Taş offers filler as the gentlest first step.
Can filler become permanent with top-ups?
Not quite, although top-ups help, because part of the earlier gel often remains. Still, the result depends on repeat visits. Read the penile filler guide for session details and glans options.
What about "permanent fillers"?
Some clinics offer PMMA microspheres, liquid silicone or oil as permanent filler. No enzyme can dissolve these substances, and they can cause hard lumps, deformity and skin loss. Therefore, Assoc. Prof. Taş never uses them, and the US FDA has not approved any filler for the penis itself.
Partly permanent: fat transfer
Fat transfer moves a small amount of your own fat, usually from the abdomen or flanks, to the shaft. Because the fat is your own tissue, it feels natural, and allergy is not a concern.
However, fat needs a new blood supply to survive. As a result, the body absorbs part of the injected fat in the first three months. The fat that survives then tends to stay, much like fat elsewhere in the body.
First 3 months: some volume fades as unsupported fat cells die.
After 3 to 6 months: the surviving volume settles and becomes stable.
Later years: large weight changes can still alter the result.
For that reason, he sometimes plans a second session to even out the shape. The penile girth enhancement guide compares fat with filler and implants in more detail.
Permanent girth: the soft silicone implant
A soft silicone girth implant sits just under the skin of the shaft. Unlike filler, it does not break down, so the added width stays. In addition, it remains fully inside the body, which keeps it discreet.
Permanence brings a different kind of risk, though. This is a real operation under general or spinal anaesthesia. So infection, fluid collection and implant removal remain possible in a small number of men.
Is the implant removable?
Yes, a surgeon can remove it in a second operation. Even so, the skin and tissue may not return exactly to their earlier shape. That is why Assoc. Prof. Taş plans the implant size carefully with each patient before surgery.
Men who also need firmer erections can combine the girth implant with a penile prosthesis. For a full comparison of sleeve brands, see our separate blog guide on silicone sleeve implants.
Permanent length: suspensory ligament release
About one third of the penile shaft sits inside the body. A ligament anchors it to the pubic bone. Ligament release divides that anchor, so more of the shaft sits outside the body at rest.
The gain itself is permanent. However, healing creates scar tissue in the gap. Without aftercare, that scar can pull the shaft back and erase part of the result.
The traction protocol decides the result
For that reason, Assoc. Prof. Taş places a small tissue barrier during surgery. Then he starts a gentle traction protocol once the wound heals.
In his practice, release with pubic contouring adds 3 to 5 cm of flaccid length in most patients. Note that the change shows mainly at rest, not during erection.
Read the penis lengthening surgery guide for the full procedure. Men who want both length and girth can review the Spartacus full enlargement package, which combines both in one plan.
Not size, but lasting function: the penile prosthesis
A penile prosthesis treats erectile dysfunction, not size. Still, men often ask about it here, so it deserves a clear note: the device gives firm erections on demand for many years. Yet mechanical parts can wear out over time, and a revision may then follow.
The prosthesis does not lengthen the penis. However, it helps keep the erect length a man still has, which untreated erectile dysfunction can slowly reduce. For more background, read our post on enlargement in men with a penile prosthesis.
What never lasts: pills, creams, exercises and devices
Online adverts promise permanent growth from pills, creams and "jelqing" exercises. No study has shown a lasting size gain from any of them. Worse, the US FDA has warned that some sexual enhancement products contain hidden drug ingredients.
Traction and vacuum devices differ slightly. Used daily for months, they can give small changes; however, those changes tend to fade once you stop. So urologists use them mainly as support around surgery, not as a cure.
Platelet-rich plasma, or PRP, also comes up often. Assoc. Prof. Taş studied PRP in men with erectile dysfunction in 2021. Even so, PRP aims at erection quality, not size, so it does not belong on a permanence list.
What can shrink a "permanent" result over time?
Even a lasting method sits inside a body that keeps changing. So a few everyday factors can reduce what you see years later.
Weight gain: a thicker pubic fat pad hides the base of the penis.
Erectile dysfunction: weaker erections can reduce erect length over time.
Scar contraction: skipped traction after ligament release can undo part of the gain.
Ageing skin: skin elasticity changes how girth looks at rest.
Fortunately, you can control most of these factors. Stable weight, regular check-ups and early treatment of erection problems all protect a good result.
Permanent or reversible: which suits you?
The right choice depends on how certain you feel about the change you want. As a rule, men who feel unsure start with a reversible step.
If you... | A sensible starting point |
Want to test how more girth feels | Hyaluronic acid filler |
Prefer your own tissue and accept some loss | Fat transfer |
Want lasting girth and accept surgery | Soft silicone girth implant |
Want more visible length at rest | Ligament release with traction aftercare |
Have weak erections as well | Erectile assessment first |
Also, many men who ask about enlargement already fall within the normal size range. In that case, Assoc. Prof. Taş says so plainly. Sometimes honest reassurance helps more than any procedure.
If worry about size takes up hours of your day, talk to your doctor first. The NHS describes body dysmorphic disorder, a treatable condition that can drive this kind of concern.
Travelling from the US, Canada or the UK
Insurers at home rarely cover enlargement, so most men pay privately. That gap makes Istanbul an option worth weighing. Even so, the NHS advises checking the surgeon's training and aftercare before any treatment abroad.
Filler: plan two to three days in Istanbul.
Fat transfer, implant or ligament release: plan five to seven days, so the first wound check happens before you fly.
Follow-up: the team checks progress by video call once you are home.

When to see a urologist
See a urologist before any enlargement treatment, and sooner in these cases.
You had an injection elsewhere and now feel lumps or hardness.
You notice redness, fever or growing pain after any procedure.
Your erections have become weaker or less reliable.
You have a bend or a lump in the shaft.
To discuss your goal with Assoc. Prof. Taş, book a consultation or message the team on WhatsApp. He will explain which methods fit your anatomy, and which do not.
Frequently asked questions
Is any penis enlargement truly permanent?
Yes. A soft silicone girth implant and a suspensory ligament release give lasting results. However, filler fades over 12 to 18 months, and part of transferred fat is absorbed in the first months.
How long does penile filler last?
Hyaluronic acid filler usually lasts about 12 to 18 months. The body breaks the gel down slowly, so many men book a top-up to keep the result.
Does fat transfer to the penis last?
Part of the fat disappears in the first few months. The fat that survives usually stays, so a second session sometimes evens out the shape.
Can a ligament release lose its length gain?
Yes. Scar tissue can pull the shaft back if you skip the traction protocol after surgery. With the protocol, the gain usually holds.
Do pills, creams or exercises make the penis permanently bigger?
No. No pill, cream or exercise has shown a lasting size gain. In fact, some sexual enhancement products sold online contain hidden drug ingredients.
Can a permanent girth implant be removed?
Yes. A surgeon can remove a silicone girth implant in a second operation. Even so, the skin and tissue may not return exactly to their earlier shape.
Does a penile prosthesis make the penis bigger?
No. A penile prosthesis restores firm erections for men with erectile dysfunction. It does not add size, although it helps keep the erect length a man still has.
