What Is Penoplasty? Procedures, Candidates and Realistic Expectations
Penoplasty is a general term for procedures that change the girth, visible length or shape of the penis. It is not one single operation. In adult men, it usually means filler, fat transfer, a soft implant, a ligament release or a mix of these.
Medically reviewed by Assoc. Prof. Dr. Tuncay Taş, urologist. Updated November 2026.
So when a clinic offers "penoplasty", your first question should be simple: which procedure, exactly? This guide explains what the word covers, who tends to benefit and where the realistic limits lie.
What does penoplasty actually mean?
The word combines "penis" with "-plasty", the medical suffix for shaping or repair. Because it describes a goal rather than a technique, clinics use it loosely. As a result, two surgeons can say "penoplasty" and mean two very different treatments.
In practice, the term falls into three groups:
Cosmetic enhancement: treatment for men who want more girth or more visible length.
Functional repair: surgery for a bend, scar tissue or a shape problem that interferes with sex.
Childhood repair: surgery for a problem present from birth, which is a separate topic from adult enlargement.
This article covers adult cosmetic penoplasty. If a bend is your main concern, see our page on congenital penile curvature surgery instead.
Penoplasty, phalloplasty and penile augmentation
Search results mix these terms, which adds to the confusion. The table below shows how each one is commonly used.
Term | Common meaning | What to check |
Penoplasty | Any procedure that reshapes the penis | Which technique the clinic means |
Penile augmentation | Adding girth, and sometimes visible length | Material used and how long it lasts |
Phalloplasty | Either enlargement or building a penis from tissue | Whether it means enlargement or reconstruction |
Ligamentolysis | Releasing the suspensory ligament | Flaccid gain versus erect gain |
Which procedures does penoplasty include?
Assoc. Prof. Taş sorts the options by the goal they serve. Girth methods work on the width of the shaft, while length methods mainly change how much shaft you see. For a wider look at each technique, read our penis enlargement techniques and safety guide.
Procedure | Main goal | Guide price in Istanbul |
Hyaluronic acid filler | Girth | $1,500 to $3,000 per session |
Fat transfer | Girth | $3,000 to $5,000 |
Soft silicone implant | Girth | $10,000 to $11,000 |
Suspensory ligament release | Visible length at rest | $3,500 to $6,000 |
These prices come from his published guide list, and a written quote follows the consultation. By comparison, the same list shows ligament release at $10,000 to $20,000 in the United States and £8,000 to £13,000 in the United Kingdom.
Anaesthesia and the length of the result also differ by method:
Procedure | Anaesthesia | How long it lasts |
Hyaluronic acid filler | Local | About 12 to 18 months |
Fat transfer | Sedation or general | Years, after early partial loss |
Soft silicone implant | Sedation or general | Permanent |
Suspensory ligament release | Sedation or general | Long-lasting, with traction aftercare |
Filler: the reversible first step
A urologist places soft hyaluronic acid gel under the skin of the shaft. The session takes about 30 to 45 minutes, and you walk out the same day.
Moreover, an enzyme can dissolve the gel if you dislike the result. For that reason, Assoc. Prof. Taş treats filler as the gentlest way to test a change. Read more in our penile filler guide.
Fat transfer: your own tissue
Here, the surgeon takes fat from the abdomen or flanks, purifies it and injects it along the shaft. Since the tissue is your own, there is no foreign material. However, the body absorbs part of the fat early on, so a second session sometimes follows.
Soft silicone implant: lasting girth
A soft silicone implant sits just under the skin of the shaft and does not break down. So the added width stays, although this remains a real operation with its own risks. Our penile girth enhancement guide compares all three girth methods, and a separate post covers silicone sleeve implants.
Ligament release: length you see at rest
A ligament anchors the penis to the pubic bone, and part of the shaft sits inside the body. Releasing that ligament lets more of the shaft hang outside. Consequently, the penis looks longer at rest, but the erectile bodies keep their size.
Scar tissue can then pull the shaft back, so traction after surgery matters. Our penile traction guide explains why. For the full operation, see the penis lengthening surgery page.
Combined penoplasty: length and girth together
Some men want both a longer look at rest and a fuller shape. A surgeon can then combine a ligament release with a girth method in one plan. However, a combined plan adds operating time and more healing to manage, so it needs a stronger reason than a single method.
Our penis enlargement surgery guide sets out the length and girth options, the stay and the guide prices in Turkey. Our Spartacus package page shows how one combined plan works. Still, the right mix always depends on your anatomy, not on a package name.
What does not count as penoplasty
Adverts often place other products beside these procedures, so it helps to separate them.
Pills and creams: no study has shown a lasting size gain from them.
Stretching exercises: they offer no proven gain, and rough technique can injure the penis.
Traction and vacuum devices: they give small changes at most, so urologists use them mainly as support around surgery.
PRP injections: they aim at erection quality, not size. Assoc. Prof. Taş published early results on PRP for erectile dysfunction in 2021.
How a urologist assesses you first
A good plan starts with measurement and conversation, not with a price list. During the visit, Assoc. Prof. Taş asks what bothers you, when it started and what result you picture. Then he examines the penis at rest and on gentle stretch.
He also looks at several features that clinics sometimes skip:
Skin: loose or tight skin limits how much girth the shaft can hold.
Pubic fat pad: a thick pad can hide shaft that already exists.
Erections: weak erections need attention before any cosmetic step.
Curvature or scar tissue: a bend changes both the method and the order of treatment.
General health: blood tests check diabetes control and clotting, and an ultrasound follows where needed.
After that, you should receive a clear plan with a written quote. You should also feel free to leave without booking anything, because a pause is a normal part of a good decision.
Who is a good candidate for penoplasty?
Penoplasty suits some men well and others poorly. A careful assessment should therefore come before any talk of technique.
Usually a good fit: healthy adults with a clear goal, mainly about girth, and realistic expectations.
Often a good fit: men who lost shape after weight change or ageing, or who want a reversible first step.
Caution advised: active infection, an untreated bleeding disorder or poorly controlled diabetes.
Caution advised: expectations that no treatment could meet, or anatomy already within the normal range.
Research consistently shows that most men who worry about size are normal. In that case, Assoc. Prof. Taş says so plainly, because honest reassurance can help more than a procedure.
Similarly, some men spend hours each day worrying about how their body looks. The NHS describes this pattern as body dysmorphic disorder, a treatable condition. Surgery does not solve it, so talk to your doctor first.
What results can you realistically expect?
Honest expectations protect you more than any technique. Generally, girth responds well to treatment, while length responds less. Most disappointment starts when a man expects a large erect gain.
If your goal is... | What is realistic | What is not |
More girth | A visible, fuller shape from filler, fat or an implant | The same result from every method |
Longer look at rest | A modest gain from ligament release and aftercare | A result without traction or follow-up |
Longer erection | Little change, since erectile bodies keep their size | A large erect gain from any method |
Firmer erections | Erectile assessment and treatment first | Better erections from cosmetic penoplasty |
Results also depend on your body. For example, skin elasticity, the depth of the shaft inside the body and the pubic fat pad all shape the outcome. A thick fat pad can hide part of the shaft, so weight loss sometimes helps as well.
Durability varies too, and our post on whether penis enlargement is permanent covers each method. To see what real outcomes look like, browse the penis enlargement before and after gallery.
Penoplasty and erectile dysfunction
Cosmetic penoplasty does not treat weak erections. In fact, men with erectile dysfunction often lose some erect length over time, and that loss can feel like a size problem. So treating the erection first sometimes changes the whole question.
When tablets and other first-line options fail, a penile prosthesis can restore firm erections. It does not count as enlargement, although a soft silicone implant can be added for girth in selected men.
What happens on the day of treatment?
The day depends on the method, yet most treatments are day cases. So an overnight hospital stay is rarely needed.
Assessment: an examination and blood tests confirm your anatomy and general health.
Anaesthesia: filler uses numbing cream or local anaesthetic, while surgery uses sedation or general anaesthesia.
Procedure: a filler session takes about 30 to 45 minutes, and surgical methods take about one to two hours.
Going home: you rest for a short while, then leave with written instructions and a direct contact.
Recovery after penoplasty, week by week
Recovery is usually steady, although filler heals faster than surgery. As a general guide:
Days 1 to 3: mild soreness and swelling, eased by simple pain relief.
Weeks 1 to 2: bruising fades, and you keep the area clean and dry.
Weeks 2 to 4: everyday activity returns, but heavy exercise and sex still wait.
From about week 6: most men can resume sex, and the shape settles.
After a ligament release, traction usually starts once the wound heals. Meanwhile, the team adjusts your schedule at each follow-up.
A few simple habits protect the result during these weeks:
Keep the dressing clean and dry, and follow the wound care you receive.
Wear supportive underwear, because it limits swelling and friction.
Avoid smoking, since it slows wound healing.
Skip massage or pressure on filler or fat unless your surgeon asks for it.
Report fever, spreading redness or worsening pain the same day.
What are the risks of penoplasty?
Every method carries risk, and you deserve to hear it plainly. Most problems are uncommon and temporary. Even so, some need a second procedure.
Swelling, bruising and temporary discomfort
Infection, which needs prompt treatment
Lumps or an uneven shape after filler or fat
Partial loss of transferred fat
Temporary changes in sensation
Implant removal, in a small number of men
A result that differs from what you expected
Permanent injectables deserve a special warning. No enzyme dissolves liquid silicone, oil or PMMA, and they can cause hard lumps, deformity and skin loss.
Furthermore, the US FDA has not approved injectable silicone for any aesthetic procedure. Our post on whether enlargement surgery is safe goes further into safety.
Questions to ask before you book
The word "penoplasty" alone tells you little. These questions help you compare clinics on facts rather than adverts.
Which exact procedure do you recommend for my anatomy, and why?
Which material will you use, and can it be removed or dissolved?
What change should I expect at rest, and what during erection?
Who performs the procedure, and what is their specialty?
What happens if I get an infection or dislike the result?
Who looks after me once I am back home?
A clear answer to each question is a good sign. On the other hand, be wary of guaranteed centimetres or pressure to decide quickly.
Travelling from the US, Canada or the UK
Health insurers rarely cover cosmetic enlargement, and the NHS does not routinely fund it either, so most men pay privately. That cost gap makes Istanbul an option worth weighing. Even so, the NHS advises checking the surgeon's training and the aftercare plan before any treatment abroad.
Filler: plan two to three days in Istanbul.
Surgical methods: plan up to about a week, so a wound check happens before you fly.
Back home: tell your own doctor about the procedure, and keep your operation records.
When to see a urologist
See a urologist before any enlargement treatment, and sooner in these cases.
You had an injection elsewhere and now feel lumps or hardness.
You notice redness, fever or growing pain after a procedure.
Your erections have become weaker or less reliable.
You have a new bend or a lump in the shaft.
To discuss your goal with Assoc. Prof. Taş, book a consultation or message the team on WhatsApp. He will explain which methods fit your anatomy, and which do not.
Frequently asked questions
What does penoplasty mean?
Penoplasty is a general term for procedures that reshape the penis. In adult men it usually means girth enhancement, ligament release or a combination of both. Some surgeons also use it for repairs that restore shape.
Is penoplasty the same as phalloplasty?
Not always. Many clinics use the two words as synonyms for enlargement. However, phalloplasty also describes building a penis from tissue, for instance after major injury or in gender-affirming surgery, which is a very different operation.
Does penoplasty increase erect length?
Usually not by much. Ligament release mainly changes how long the penis looks at rest, because the erectile bodies keep their size. For that reason, men whose only goal is a longer erection are often disappointed.
Is penoplasty permanent?
It depends on the method. A soft silicone implant and a ligament release can last, while hyaluronic acid filler fades over about 12 to 18 months. In addition, part of transferred fat is absorbed in the first months.
How long is recovery after penoplasty?
Most men return to desk work within days to about a week. Heavy exercise waits for several weeks, and sex usually waits for about 6 weeks. Filler recovery is shorter than recovery from surgery.
Is penoplasty safe?
No procedure is free of risk. Swelling, infection, lumps, uneven shape and changes in sensation can all happen. Your risk also depends on the method, the materials and the training of the surgeon.
How much does penoplasty cost?
The cost depends on the method. With Assoc. Prof. Taş in Istanbul, guide prices run from $1,500 for a filler session to $11,000 for a silicone implant. Health insurance rarely pays for cosmetic enlargement.
